How to use these PDFs (so nothing slows you down)
A few simple steps can save a lot of back-and-forth later:
Download the form you need, complete it carefully, and make sure every required section is filled in before you send it back or bring it with you. If a form asks for signatures, initials, dates, or specific details, take an extra moment to double-check—those are the most common “oops” items that delay check-in.
If you’re completing forms on a phone or tablet, make sure your final version is clear and readable. A clean, legible document helps us keep your training on track.

Which form do I need?
Most divers only need one or two of these—depending on what you’re doing next.
- Training or class paperwork: start with the Dive Magic Form.
- Diving with a minor: look for the Kids Info Sheet.
- Enriched Air (Nitrox): you’ll want the Enriched Air Form.
- Leveling up beyond your current certification: check Continuing Education.
- Health-related screening: use the Medical Statement.
